• Hazardous Materials Test, for CDL, that has an "H" or "X" endorsement.

Renewal of Valid in Florida Only Driver License

• Your expired Florida License and your current Out of State License.
  
  
• Vision Test.

Lost Florida Driver License or Lost Florida ID Card

• One form of identification. Something with your name on it, as long as it is NOT a handwritten note.

Note: If your Driver License or ID Card expires for more than one year, it may not be possible for us to find it on our system. If your lost, Driver License or ID Card is expired, you may be required to comply with requirements pertaining to someone that has never had a license.
